Subject: Authorization Procedures for Radioactive Materials
Authority: Radiation Safety Officer
- Purpose. Users of radioactive materials must be approved by the Radiation Safety Committee (RSC). This procedure prescribes the approval process.
- Policy. The University of Maryland Radiation Safety Committee will act upon all new and renewal applications for possession and use of radioactive materials.
- Procedure.
New Applicants:
- Issue the following information (handouts) upon receipt of a request for authorization to possess and use radioactive material:
- Application Forms (Print with a font size of 10)
- Training and Experience Form for Users of Radioactive Materials
- Dosimetry Service Card [pdf format] [word format]
- Memo to new users
- Radiation Safety Manual
- Film Badge Information
- Licensing Procedure Flow Chart
- Action taken upon receipt of information from applicant:
- Check that items 1-17 on the application are satisfactorily completed.
- Check for proper signatures on the application.
- Check that the training and experience form is complete.
- After review of forms, contact the applicant if there are any questions which require further information.
- Prepare file and begin checklist of status of information.
- Contact applicant and set up appointment for pre-licensing survey inspection.
- Complete the pre-authorization and inspection forms, and prepare laboratory map for survey purposes.
- Make recommendations as necessary.
- Submit all forms to the Radiation Safety Officer and/or Radiation Safety Committee Chairman for interim approval.
- Issue Interim Authorization.
- Assign Authorization No. Check existing numbers and assign next number in sequence.
- Expiration date will be two years from the date of committee approval.
- Enter authorized use of material.
- List any conditions placed on the user's authorization on page two of the authorization.
- If application is not approved, inform requestor of the reason and attempt to get any problems resolved.
- If approved, send a copy of the Interim Approval to user.
- Issue the following information (handouts) upon receipt of a request for authorization to possess and use radioactive material:
- Final Authorization.
After review and approval by the Radiation Safety Committee, issue Authorization.
- Renewal Application.
- Two months prior to the expiration date of an authorization, send a renewal form to the principle investigator.
- Whether there are any changes requested or not, the renewal application information will be submitted to the RSC for approval.
- Timely renewal of authorization will be granted by the Radiation Safety Officer and/or RSC Chairman until the RSC committee meets.
- Upon approval by the committee, a new authorization must be issued. Change only the items noted on the renewal request.
- Amendments to an Authorization:
- Any changes or additions to an authorization must be requested in writing.
- The Radiation Safety Officer and/or the RSC Chairman can issue an Interim Approval for an amendment.
- The changes will be reviewed by the RSC for approval at the regular meeting.
- If approved by the committee, issue an amendment to the authorization.
